I observe them for a long time before drawing. I position myself on an angle where they can't easily see me and draw one feature at a time stopping every time they look yp

Profiles are easier than front views because you're in the blind spot of the subject. In order for them to know what I'm doing they'd have to turn their head and I'd see that first and be able to stop drawing.
